Dear Client,

I hope you are settling into married life.

I also hope that you might find a couple of minutes to provide feedback about my services as your marriage celebrant.

It is always my intention to provide a professional service that reflects the importance of the day for you, meets your hopes for the day, and complies with the legal requirements for marriage.

I hope that has been the case for you. If you have ideas for improvement I would like to hear those too.

Please complete the fields below at your earliest conveince.

01 Question
Overall, how would you rate my performance as your celebrant - thinking from our first contact, our discussions and planning, the production of paperwork, officiating at your wedding until we finally signed the Marriage Certificates
02 Question
How do you rate our first meeting, when we discussed your ideas and desires for the ceremony, viewed options for ritual and words, and pulled together a plan?
03 Question
How do you rate the rehearsal? Was it conducted smoothly? Did it provide clarity for the wedding party who participated?
04 Question
Did your wedding ceremony match your expectations?
Was there anything that I did that you would recommend that I offer/provide on other occasions?
Do you have suggestions for improvements?
07 Question
I like to include photos of wedding ceremonies that I have officiated at, on my business Facebook page, and website. Are you able to send me one or two photos from the wedding ceremony, for that purpose? (You can email them to me if you like andrew@andrewsmithlifeevents.com)
I also include quotes from clients - from this feedback form or other correspondence - on the website. Do you have additional feedback?

Thanks for completing this feedback form.

My postal address is 106 Lewisham Scenic Drive Lewisham Tasmania 7173.

Good luck with married life. May the future be wonderful for you both.

May the best you have ever seen, be the worst you will ever see.

Warmest Regards

Making a complaint

I hope that my services as Marriage Celebrant reflected the importance of your marriage and the way in which you wished to conduct your wedding.

If however, you believe my conduct has been unprofessional, you are able to make a formal complaint.

The Attorney Generals Department advises:

If you want to make a complaint about a Commonwealth-registered marriage celebrant, you should consider:

· whether you can settle the matter directly with the celebrant

· that we do not have the power to make a celebrant refund money

· that your complaint may be refused if it is not made within three months of the event.

You can email us at marriagecelebrantssection@ag.gov.au to request more time.
